How to Improve Patient Safety in Primary Care
A series of presentations on the importance of safety in primary care, safety culture, diagnostic error and the epidemiology and prevention of medication error.
In October 2010 we held a conference on patient safety for primary care clinicians (GPs, nurses, pharmacists and professions allied to medicine) from EU member states. Our task was to introduce key concepts in patient safety to clinicians who had little knowledge or experience of patient safety in primary care. We wanted to explain why patient safety was important in the primary care context, what we meant by safety culture and how it could be measured, the importance of diagnostic error and the epidemiology and prevention of medication error. We recorded the talks and presentations which were given by leading experts and hope that they can be now be used a resource by people who could not attend the conference but who are keen to develop initiatives related to patient safety in primary care in their own countries. The aim of the LINNEAUS collaboration is to increase knowledge and develop practical action to improve patient safety and we hope that these presentations will be an important resource for clinicians and researchers.

- The Importance of Patient Safety in Primary Care
Professor Aneez Esmail - University of Manchester
In the opening presentation of the conference, Professor Esmail explains why patient safety in primary care is important. Through the description of his own clinical practice he gives examples of the challenges faced by primary care clinicians and describes a plan of action for ensuring that patient safety becomes a central feature in the delivery of primary care.’ (approx. 40 minutes).

Professor Dianne Parker - University of Manchester
What is safety culture? How can it be measured? Dianne Parker a psychologist and leading expert on safety culture, describes why understanding and improving safety culture is essential to improving safety culture. She describes the use of the Manchester Patient Safety Framework (MaPSaF) in primary care. (approx. 30 minutes)

- Diagnostic Error and Patient Safety
Dr Olga Kostopoulou - King’s College, London
Why is diagnostic error such an important cause of error in the primary care setting? Olga Kostopoulou one of the foremost experts on diagnostic error in primary care in the UK, describes how everyday clinical reasoning results in diagnostic error and describes the research that she is carrying out to understand one of the most important cause of errors in primary care. (approx. 30 minutes)

- The Epidemiology of Medication Error in Primary Care
Andreas Soennichsen - Paracelsus Medical University, Austria
Medication error is the most common form of error in primary care and the type of error that most physicians and pharmacists are aware of. Understanding the causes of medication error is therefore a prerequisite for preventing a major cause of harm to patients. In this presentation, Andreas Soennichsen, Professor of Medicine with a special interest in quality of care and medication error, describes the epidemiology of medication error in the primary care setting. - What can we Do About Medication Error in Primary Care?
Miriam Lainer - Paracelsus Medical University, Austria
In a linked presentation on medication error, Dr Miriam Lainer describes the outcome of the literature review carried out by colleagues at Paracelsus University which looked at the evidence for improving medication error in patient safety. There is now good evidence combined with practical action that can enable significant improvements in medication safety across all areas of primary care. (approx. 20 minutes)

- Patient Safety in an International Context
Susan Dovey - Dunedin School of Medicine, University of Otago, New Zealand
Susan Dovey, an Associate Professor of Health Services research, is one of the foremost researchers in the field of patient safety in primary care. Together with colleagues in North America, she was the founder of the original LINNEAUS collaboration and designed and carried out the groundbreaking study of patient safety in primary care in the late 1990’s. She continues to carry out research in the area and is an active member of the WHO Alliance for Patient Safety. In this presentation she sets out the challenges for patient safety in primary care in an international context, drawing on her experience as a researcher in North America, Australia and New Zealand. - So What can we Do Now?

In this presentation Professor Esmail summarises the key points from the conference, the lessons that we can take away and sets out the future work plan for the LINNEAUS collaboration for the new members who will now have joined the collaboration. (approx. 15 minutes)
